My VoIP calls are fading and dropping. Where should I start to troubleshoot this problem?

The first thing I'd check is the performance of the end-to-end IP service -- primarily packet loss and jitter. If you are experiencing more than 1% packet loss end-to-end or 10msec of jitter end-to-end you're likely to experience problems. Many service providers do not cover packet loss but most do not cover jitter. If you believe your IP network performance (end-to-end) is fine, then I'd check to make sure there aren't any configuration issues or whether there is a newer version of software that may have some fixes.
