Softswitches use an interface to the PSTN. When a call is not available over IP, it will switch to the PSTN. The PSTN gateway may or may not be at your location. There are companies out there that allow you to use their interface and all you buy is the softswitch and Internet connection. The interface places the call over the PSTN and maintains the call through completion.
